Long Term Conditions

Long Term Conditions (LTC) are designed to ensure that we provide the most appropriate and up to date care and they're run at a frequency to allow patients to be reqularly reviewed.

Test Results

If your test results show that you need more tests or treatment, we will contact you.

Once a clinician has reviewed your test results, you can view them.

Online Triage

There are many things you can do online at our surgery:

  • request help about a non-emergency medical problem
  • order repeat prescriptions
  • request a sick note
  • ask about test results
  • contact us about an admin request

Social prescribing

A social prescriber creates your own social prescription to provide support that will help you access activities to improve health and enjoyment in life.

Group clinics

Group clinics are a way for you to spend more time with your specialist or community doctor, advanced care practitioner, nurse or GP, get your one to one review and get answers to your questions alongside others living with similar health issues.

Vaccinations

This guide can help you understand the vaccines offered in the UK and when to have them. It also explains how they work and why they're safe and important. 

Getting a fit (sick) note

Request a sick note if you've been sick for more than 7 days.

Private services

We are happy to undertake, by appointment, medical examinations, reports for insurance companies, employers and solicitors. We can carry out fitness medicals for sports, elderly drivers etc.

Travel Clinics

If you're planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world.

Appointments

If you need medical assistance at Beech Tree Surgery, you can access this in a number of different ways. We want to ensure a safe and fair access to our appointments for all patients registered with us. 

Self-referral

Refer yourself to a health service such as talking therapies without having to see a GP first.

Repeat Prescriptions

The easiest ways to order repeat prescriptions are:
